What is a Certificate of Authenticity?
The Certificate of Authenticity is provided with your Crizal purchase. You must include this a copy of this certificate in order to qualify for your rebate.
My rebate submission was rejected. What do I do?
Rebates can be rejected for several reasons. Some will allow for resubmission and others will not. If a rebate is rejected, you will receive a notification indicating why your rebate was rejected and what to do next.
If your rebate submission did not include required items such as proof of purchase, Certificate of Authenticity, etc., you will be asked to send the required information to continue with processing.
If your rebate submission was rejected for reasons that disqualify the submission, you will not be allowed to resubmit. Some examples of reasons where resubmission may not be allowed could include:
- Purchase was not made within the required timeframe
- Rebate submission was not postmarked by the required date
- Purchase was made from a retailer who was not participating in the rebate promotion
- You already submitted a rebate for the promotion and multiple submissions are not allowed
- You purchased a product which does not qualify for the promotion
- Do I have to send the original receipt or can I send a copy?
This is determined by the terms and condition of the rebate itself. Please refer to the rebate form for complete details on terms of submission. Regardless of whether you are required to send the original or a copy, you should keep a copy of the receipt for your records.
- I have lost my original receipt.
Many stores have the ability to reprint receipts. You should contact the retailer where you made your purchase to request a duplicate.
Where is my additional $10 rebate.
In order to qualify for the additional $10 rebate offer, you are required to visit www.thinkaboutyoureyes.com/summer and enter the rebate code on your form. This code will generate an additional form you were asked to mail in with your original rebate. If we received your submission without this form, you will not qualify for the additional $10 offer.
I made a purchase and there are multiple rebates available but I only have one receipt
Please check your rebate form. If you can qualify for multiple rebates, you may be able to complete and submit your product rebate forms for several rebates at the same time with your single receipt.
